How to Play: Bring your game card into Fidelity’s West Scranton branch on or before September 30, 2008 where you will find the Prize Decoder Display. Insert your game card in the appropriate slot. If your game card lights up a light next to a listed prize, you win that prize (subject to verification). You may also obtain an official game card upon request from the West Scranton branch through September 30, 2008 while supplies last. Limit of one (1) game card per household.
Prizes and Odds of Winning: First Prize (1 winner): 2008 Lexus RX 400H; Second Prize (1 winner): $10,000 travel voucher from Adventure Travel and Tours 405 S Main St, Scranton PA; Third Prize (3 winners): High Definition(HD) Television valued up to $1,000; Fourth Prize (700 winners): Portable Lawn Chair; Fifth Prize (2,500 winners): Portable flashlight and tool kit combination. Odds of Winning may vary. Taxes for any prize are the sole responsibility of the winner.
How to Claim Prizes: Provide your winning ticket to a Fidelity representative in the West Scranton branch on or before September 30, 2008 whom will verify your prize. The branch representative will provide Grand Prize, Second Prize, and Third Prize winners information on how to redeem their prize. All other winners may claim their prize at the West Scranton branch. Prize claims are subject to verification and availability. The individual redeeming a prize shall be the same individual identified on the winning game card.
Eligibility: Sweepstakes open to legal residents of Pennsylvania who are 18 years of age or older as of September 30, 2008. Employees of Fidelity and its respective branches, affiliates, subsidiaries, suppliers, advertising and promotional agencies and the immediate family members or household members of such employees are not eligible to participate. Fidelity agents, independent contractors, attorneys, settlement service providers, including, but not limited to, real estate agents and brokers, appraisers, property insurance providers and escrow agents are not eligible to participate. Limit one (1) prize per household. Prizes are non-transferable and no substitutions allowed except at sole discretion of Fidelity. Void outside of Pennsylvania wherever prohibited. All federal, state, and local laws apply.
Conditions: First, Second, and Third Prize potential winners will be required to sign and return an affidavit of eligibility and liability/publicity releases within 14 days of notification or the prize will be forfeited. All prize claims and game materials submitted become the property of Fidelity and will not be returned. All winners, by acceptance of a prize, agree to release Fidelity and its respective branches, affiliated, subsidiaries, employees, officers, directors, suppliers, distributors, advertising and promotional and judging agencies from any and all liability for injuries, losses or damages sustained in connection with participation in this sweepstakes or the possession, acceptance, or use of any prize awarded in this sweepstakes. By accepting prize, winners also consent to the use of their names, pictures, and other likeness for publicity and promotional purposes as determined by Fidelity without additional compensation. Sweepstake materials are automatically void if they are forged, altered, mutilated, or tampered with in any way; or if they are obtained outside legitimate channels; or if they contain printing, production, mechanical, typographical or other errors. Liability for sweepstake materials containing printing or other errors is limited to replacement with other such materials while supplies last. Fidelity is not responsible for damaged, lost, late, illegible, incomplete, misdirected, or postage due prize claims or mail. No more than the stated number of prizes will be awarded. In the unforeseen event of a duplicate grand prize winner, one (1) winner shall be selected from a random drawing as the grand prize winner. Any attempt to forge sweepstakes materials or defraud in any way will be prosecuted to the fullest extent allowable by law. Fidelity has the sole power and absolute discretion to terminate the sweepstakes at any time, for any reason and without prior notice, to interpret the rules and make decisions regarding the interpretation of the rules.
